Sir,?When sloughing of the flap occurs afrer Syrne's operation nt the ankle-joint, it is said to be invariably the fault of the operator. It appears to me, however, that, when a surgeon is fully alive to all the causes which are known to produce sloughing, and operates with ordinary skill and care, there surely must be some unavoidable factor at work when sloughing occurs.
